Pileus - conical when young, becomes convex then    applanate when mature, 4 - 22 mm diameter, wheat brown to corn silk brown in color, center is slightly elevated,  surface smooth and glabrous.

Stipe - cylindrical, 29 mm long, equal, central, solid, surface white, glabrous

Spores - hyaline

Ecology - mutualistic symbiont with termites, usually found on termite mound

Occurrence - Lukong Valley, Dolores, Quezon
